Among other things, the appendices to the Rules of Procedure were amended:
- a template for a request to the Committee for an opinion on conducting a study – constituting Appendix No 3 to the Regulations,
- a template for an informed consent form for a study participant/legal representative of a study participant – Appendix No. 2 to the Regulations.
The new templates are also available on the Committee's website (English language versions to be published soon)
The changes were introduced by the Regulation of the Rector of the University of Lodz on 15 December 2025 (in Polish)
