What details will appear on the diploma?
The diploma will include, among others:
- first name(s),
- surname,
- date of birth,
- place of birth.
Sounds simple? It may seem so, but this is exactly where mistakes most often occur.
I have two first names. Does it matter?

If you have two first names in your official documents, make sure that both are correctly entered in the system. The diploma must contain details consistent with your official documents.
I changed my surname during my studies. What then?
If you changed your surname during your studies and have not reported it, be sure to contact the Dean’s Office.
The diploma will be issued based on the data in the system, so you should make sure that it is up to date.
Place of birth is not the same as place of residence
This is one of the most common mistakes.
If you were born in Piotrków Trybunalski but live in Łódź, your diploma should state Piotrków Trybunalski, not Łódź.

It is just one letter, but it matters on your diploma. The same applies to an incorrect date of birth or a missing second name.
