Submitting an application
The application form and its attachment should be downloaded from the “Research Ethics Committee” tab on the university’s website or from the Employee Portal – Knowledge Database, filled in, signed by the required parties, scanned and sent to: komisja.etyka@uni.lodz.pl
Applications must be submitted electronically only.
